In order to effectuate the Connecticut Dental Health Partnership's mission of enabling all HUSKY Health members to achieve and maintain good oral health, we are committed to ensuring that access and availability of oral health services are fairly distributed across the CT Medicaid population. Achieving oral health equity requires us to develop data driven strategies to address population specific needs to reduce barriers in accessing and utilizing oral health services.

It is important to have a Dental Home

Everyone should have a Dental Home. "Dental Home" means an ongoing relationship between a dentist and a patient, inclusive of all aspects of oral health care delivered in a comprehensive, continuously accessible, coordinated and person or family-centered way. Establishment of a dental home for a patient should begin no later than 12 months of age or within six months of enrollment in the CT Medical Assistance Program and include referral to dental specialists when appropriate. The key features of the dental home include:
  • Provides comprehensive dental care including prevention and emergency services by a Connecticut Medicaid enrolled licensed dentist at a fixed location;
      "Comprehensive Dental Care" is the coordinated delivery of dental services that improves or maintains their oral health to a functional level. It includes the adjunctive, diagnostic, endodontic, oral surgical, orthodontic, pediatric, preventive, prosthodontic, restorative and emergency services available under the program. Regular preventive recall appointment reminders are performed for all patients of record. Comprehensive dental care is personalized to each patient and includes individualized oral care instruction.
  • If needed services are not available, there is formal written or electronic referral to another appropriate dental provider or specialist and all services continue to be coordinated by the dental home;
  • Care is coordinated to maximize effectiveness and efficiency, consistent with evidence-based and community standards;
  • Completes a disease risk assessment for each patient and uses it to design an individualized treatment plan. It must include consideration of the patient's overall health and any needed coordination with other health care providers;
  • Is continuously accessible ;
    • It has a fixed location for the continuity of services that is within a twenty mile radius of the patient base. Exceptions may be made by at the discretion of the Department of Social Services in cases of rural underserved areas as defined by the CT Department of Public Health;
    • Records of treated patients are available at that site.
    • Charts and records of treated patients are accessible at that site.
    • Has regularly scheduled appointment hours available weekly.
  • Is person or family centered;
    • For primary care dental providers which are not pediatric or geriatric specialists:
      • Accepts patients starting at age one with no upper age limit;
      • Accepts both adult and child patients;
      • Encourages all members of the family to have the same dental home;
      • Maintains the patient's right to choose their and their (and their families') Dental Home;
      • Encourages patients to stay with their chosen Dental Home for an extended period of time;
    • For primary care dental providers which are pediatric or geriatric specialists :
      • Encourages other family members to have dental home(s) and makes referrals as needed;
      • Maintains the patient's right to choose their (and their families') Dental Home;
      • Encourages patients to stay with their chosen Dental Home for an extended period of time;
  • Has a plan for providing emergency care after regularly scheduled office hours twenty-four hours a day, seven days per week, other than simply providing a referral to the local hospital emergency room;
  • Has written or electronic communication with primary care and specialty medical providers when in the best interests of the patient;
  • A mobile or remote program must meet these criteria;
    • Has a home office at a fixed location within twenty miles of the patient base. Exceptions may be made by at the discretion of the Department of Social Services in cases of rural underserved areas as defined by the CT Department of Public Health.
    • Only submits claims for services provided within a geographic area that is not greater than 20 miles of the fixed location. Exceptions may be made by at the discretion of the Department of Social Services in cases of rural underserved areas as defined by the CT Department of Public Health.
    • Accepts patients from the mobile or remote program at the fixed location as a patient of record, provides comprehensive dental care and keeps the charts of treated patients at the fixed location.
    • Otherwise meets the definition of a "Dental Home".
