Fee Schedule has been updated effective January 1, 2016. - Updates have been made to the dental fee schedule effective January 1, 2016. Some Procedure Codes have been end-dated as of 12/31/2015. New Procedure Codes have been added as of 1/1/2016. Please refer to the dental fee schedule for prior authorization requirements and reimbursement rates. A Policy Bulletin with these updates has also been published.

Fluoride Code D1206 Added to Dental Fee Schedule - Effective 9/1/2015 The fluoride code D1206 has been added to the dental fee schedule. Please disregard our guidance from 2013 regarding the billing of dental procedure codes D1206 and D1208. Please bill these codes based on the description in the CDT 2015 or the most current version in the future. The guidelines that describe the benefit for these procedures can be found in our most recently updated benefit grid found under the tab for Provider Reference Material.

Guidelines For Scoring Orthodontic Cases - The Connecticut Dental Health Partnership uses the J.A. Salzmann, Handicapping Malocclusion Assessment Record to determine the severity of malocclusion. The completed assessment record is used to calculate an index number ("Salzmann Scoring Index") which is one criteria used in the evaluation of the orthodontic case.
